Amoxicillin is not the first-choice treatment for most people with chlamydia, but medical guidelines list it as an alternative during pregnancy.

Chlamydia can be treated with antibiotics, but amoxicillin is not the first-choice medicine for most people with the infection. Treatment recommendations differ for pregnant women and depend on the patient’s circumstances.

The infection is caused by Chlamydia trachomatis, a bacterium transmitted mainly through sexual contact. Many infected people have no symptoms, which means the condition can go undetected without testing.

Untreated chlamydia can cause complications, including pelvic inflammatory disease, ectopic pregnancy and infertility in women. Testing and appropriate treatment help reduce these risks.

Which antibiotics are recommended?

The US Centers for Disease Control and Prevention (CDC) recommends doxycycline as the preferred treatment for chlamydia in most adolescents and adults. The standard course is 100 milligrams taken twice daily for seven days.

Azithromycin, usually administered as a single 1-gram dose, is an alternative in certain circumstances. A healthcare professional can determine which treatment is appropriate based on the patient’s medical history, pregnancy status and other factors.

Amoxicillin belongs to the penicillin group of antibiotics. Although it is used to treat several bacterial infections, it is not the preferred treatment for chlamydia in most non-pregnant adults.

However, it is incorrect to say that amoxicillin cannot treat chlamydia because the bacteria have universally developed resistance to penicillin antibiotics.

When is amoxicillin used for chlamydia?

Pregnancy changes the choice of treatment. The CDC recommends azithromycin as the preferred antibiotic for pregnant women diagnosed with chlamydia and lists amoxicillin as an alternative.

When prescribed as an alternative, the CDC regimen for pregnant patients is amoxicillin 500 milligrams taken three times daily for seven days. Patients should follow their healthcare provider’s instructions rather than use this dosage without a diagnosis and prescription.

Pregnant women should also undergo a test to confirm that the infection has cleared approximately four weeks after completing treatment. Further testing is recommended about three months later.

Testing and preventing reinfection

People diagnosed with chlamydia should be tested for other sexually transmitted infections, including gonorrhoea, HIV and syphilis. Their sexual partners may also need testing and treatment.

Patients should avoid sexual activity for seven days after single-dose treatment or until they have completed a seven-day antibiotic course and their symptoms have resolved. They should also wait until their partners have received treatment.

Most non-pregnant patients do not routinely require a test to confirm that treatment has worked if they have taken their medication correctly and have no continuing symptoms or suspected reinfection. However, the CDC recommends retesting approximately three months after treatment because reinfection is common.

Anyone who suspects they have chlamydia should seek medical assessment rather than self-treat with amoxicillin or leftover antibiotics. Testing can establish whether the infection is present and help determine the appropriate medicine.

The key point: Doxycycline is the preferred treatment for most non-pregnant adults, while azithromycin is the preferred option during pregnancy. Amoxicillin is an alternative for pregnant patients under clinical guidance, not a general first-line treatment for everyone with chlamydia.
