Does Omeprazole Cause Weight Gain? What the Evidence Shows

Omeprazole is widely used to treat acid reflux and heartburn, but evidence that it directly causes weight gain remains limited. Here is what to know about its side effects and long-term use.

Find out whether omeprazole causes weight gain, which side effects are recognised, what long-term use may mean and when to consult a healthcare professional.

Omeprazole, a medicine used to reduce stomach acid, has been associated with weight gain in some reports, but the available evidence does not establish it as a common or predictable effect of treatment.

The drug belongs to a class of medicines known as proton pump inhibitors (PPIs), which reduce the amount of acid produced in the stomach. It is used to treat conditions including gastro-oesophageal reflux disease (GERD), heartburn and stomach ulcers.

Weight gain is listed among reported adverse reactions in some omeprazole prescribing information, although its frequency and relationship to the medicine are not firmly established. The UK National Health Service lists headache, stomach pain, nausea, vomiting, constipation and diarrhoea among the common side effects, but does not identify weight gain as a common reaction.

Why weight may change during treatment

Several factors can contribute to changes in weight while a person is taking omeprazole.

The medicine may relieve heartburn and other symptoms that make eating uncomfortable. As symptoms improve, a person may return to their usual eating pattern. However, this does not establish that omeprazole directly increases appetite or causes fat gain.

Weight changes can also occur for reasons unrelated to the medicine, including changes in diet, activity, other medications or an underlying health condition. A change in weight during treatment does not, by itself, prove that omeprazole is responsible.

Swelling can also affect the number on the scale. Unlike an increase in body fat, swelling involves fluid accumulating in body tissues. New or unexplained swelling, particularly when accompanied by rapid weight gain, should be assessed by a healthcare professional.

Swelling of the face, lips, tongue or throat, especially with difficulty breathing, may indicate a serious allergic reaction and requires emergency medical attention.

Other side effects and long-term risks

Common side effects of omeprazole include headache, abdominal pain, nausea, vomiting, diarrhoea, constipation and increased gas. Many are mild, but persistent or severe symptoms should be discussed with a healthcare professional.

Long-term use can carry additional risks. Prescribing information warns that prolonged treatment may contribute to vitamin B12 deficiency because reduced stomach acid can interfere with the absorption of the vitamin from food. Rare cases of low magnesium levels have also been reported, sometimes after several months of treatment and more commonly after a year or longer.

Low magnesium can cause muscle cramps, weakness, fatigue and, in severe cases, abnormal heart rhythms. People who require prolonged treatment, particularly those taking certain other medicines, may need monitoring based on their medical circumstances.

Long-term proton pump inhibitor use has also been associated with an increased risk of bone fractures, particularly with high doses or treatment lasting a year or more. However, an association does not necessarily establish that the medicine directly causes fractures in every person who takes it.

Omeprazole has also been associated with Clostridioides difficile infection, which can cause persistent diarrhoea. Severe or continuing diarrhoea during treatment warrants medical assessment.

How long does omeprazole take to work?

Some people begin to notice an improvement in symptoms within two to three days, although full relief can take longer. The time required depends on the condition being treated and its severity.

Treatment duration also varies. Some people need the medicine for a short course, while others require longer treatment for conditions such as severe reflux-related damage to the oesophagus.

People taking prescription omeprazole should follow their prescribed regimen and discuss prolonged use with their healthcare professional. Those using non-prescription products should follow the package instructions and seek medical advice if symptoms persist or worsen.

Lifestyle changes that may help with acid reflux

Lifestyle adjustments can help reduce symptoms of gastro-oesophageal reflux disease, although individual triggers vary.

  • Meal timing: Avoid eating within two to three hours of going to bed.
  • Sleeping position: Raising the head of the bed can help people who experience reflux at night.
  • Food triggers: Identify foods and drinks that consistently worsen symptoms rather than eliminating broad food groups unnecessarily.
  • Meal size: Smaller meals may help some people manage symptoms.
  • Weight management: For people who are overweight, weight loss may improve reflux symptoms.
  • Smoking: Avoiding tobacco can help reduce reflux-related symptoms.

These measures may complement medication but do not replace treatment when it is medically necessary. The American College of Gastroenterology recommends taking proton pump inhibitors 30 to 60 minutes before a meal for GERD symptom control.

Anyone experiencing unexplained weight changes, persistent digestive symptoms or suspected medication side effects should seek advice from a healthcare professional. Omeprazole should not be stopped or changed solely because of a suspected association with weight gain without first discussing the decision with the prescribing clinician.
