Hydroquinone is a skin-lightening ingredient used to treat dark spots and uneven pigmentation, but improper or prolonged use can cause skin irritation and, in some cases, lasting discoloration. The US Food and Drug Administration (FDA) warns that over-the-counter skin-lightening products containing hydroquinone can pose health risks and are not approved for sale in the United States.

The ingredient works by reducing melanin production. Melanin is the pigment responsible for skin colour. Hydroquinone can help treat conditions such as melasma, which causes patches of darker skin, and post-inflammatory hyperpigmentation, in which dark marks develop after skin inflammation or injury.

Although hydroquinone can be effective for these conditions, its safety depends on the formulation, how it is used and the duration of treatment. Dermatology guidance recommends appropriate monitoring because irritation and other adverse effects can occur.

What are the side effects of hydroquinone?

Common adverse effects include redness, dryness, stinging, burning and skin irritation. Some people may develop contact dermatitis, an inflammatory reaction that can cause a rash. The FDA has also received reports of facial swelling and other serious skin reactions associated with hydroquinone-containing skin-lightening products.

A more serious concern is exogenous ochronosis, a condition associated with hydroquinone use that causes blue-black or grey-blue skin discoloration. A systematic review of 126 reported cases found that the condition was frequently associated with prolonged use, with a median reported duration of five years. The review also identified cases involving concentrations above 4%, although the evidence does not establish a single exposure threshold below which the condition cannot occur.

The discoloration can be difficult to treat. Anyone who develops unusual darkening or a change in skin colour while using hydroquinone should stop using the product and seek medical assessment.

Why are some skin-lightening products dangerous?

The risks associated with skin-lightening products extend beyond hydroquinone. Some products contain mercury, a toxic metal that can damage the kidneys and nervous system. The World Health Organization (WHO) says mercury-containing skin-lightening products remain in use in several regions, including Africa.

The FDA has also found skin-lightening products containing high levels of mercury or hydroquinone, including products in which the ingredients were not properly disclosed. This means a label may not always provide a complete picture of a product’s contents.

Consumers should be cautious about products with missing ingredient lists, unclear labelling or uncertain origins. Checking the label and buying from a reputable source can help, but neither guarantees that a product is safe.

Is hydroquinone banned?

Hydroquinone regulations vary between countries.

In the United States, the FDA does not approve hydroquinone-containing skin-lightening products for over-the-counter sale. Prescription products containing the ingredient remain available through healthcare providers. The FDA also warns consumers against using unapproved over-the-counter skin-lightening products.

These restrictions should not be described as a worldwide ban. The regulatory position differs by jurisdiction, and Uganda’s current rules would need to be confirmed with the National Drug Authority or the relevant regulator before making a definitive claim.

How can hydroquinone be used more safely?

Hydroquinone treatment should be guided by a qualified healthcare professional who can assess the cause of the pigmentation and determine whether the ingredient is appropriate. Dermatology guidance describes limited treatment courses and recommends stopping treatment if significant irritation develops.

Anyone experiencing a significant skin reaction or unusual blue-black discoloration while using hydroquinone should stop using the product and seek medical advice. The FDA also advises consumers to consult a healthcare provider about treatment options for dark spots and other skin conditions.

Hydroquinone is not inherently unsafe in every medically supervised use, but it is not risk-free. It can help treat certain pigmentation disorders, while inappropriate or prolonged use can cause adverse effects, including potentially lasting skin discoloration. Unregulated skin-lightening products introduce additional risks, particularly when they contain undisclosed or hazardous ingredients.
