Learn about Ureaplasma urealyticum symptoms, testing and treatment, including when antibiotics may be needed and why a positive test does not always mean infection.

The bacterium can live in the genital tract without causing illness, and experts caution against routine testing and antibiotic treatment in people without a clear clinical indication.

Ureaplasma urealyticum is a type of bacterium that can be present in the urinary and genital tracts of healthy people without causing symptoms. Its detection in a laboratory test does not necessarily mean a person has an infection requiring treatment.

The bacterium has been associated with inflammation of the urethra, the tube that carries urine out of the body, particularly in men with high bacterial levels. However, evidence linking it to a range of reproductive health problems remains mixed, making clinical assessment important before treatment is considered.

A position statement published by the European STI Guidelines Editorial Board in 2018 advised against routine testing and treatment for U. urealyticum and related bacteria in men and women. The authors said there was insufficient evidence that routinely detecting and treating these bacteria produced more benefit than harm.

Symptoms and when to seek medical care

Many people carrying U. urealyticum have no symptoms. When symptoms occur, they may include pain or burning during urination, itching around the urethra and unusual genital discharge.

These symptoms are not specific to U. urealyticum. They can also occur with other sexually transmitted infections (STIs), urinary tract conditions and other genital infections.

The US Centers for Disease Control and Prevention (CDC) says evidence linking Ureaplasma species to urethritis is inconsistent. Most men with Ureaplasma detected do not have clear signs of disease unless bacterial levels are high. The CDC does not recommend testing women for U. urealyticum as part of routine investigations for cervicitis, or inflammation of the cervix.

People experiencing persistent pain when urinating, unusual discharge, genital discomfort or lower abdominal pain should seek assessment from a healthcare professional. Fever or severe pelvic or abdominal pain warrants prompt medical attention.

A clinician can assess the symptoms and determine whether tests for established causes of infection, including chlamydia and gonorrhoea, are needed.

How is Ureaplasma detected?

Diagnosis depends on the clinical circumstances. Laboratory testing may involve a urine sample or a swab from the affected area. Molecular tests can detect bacterial genetic material, but a positive result alone does not establish that the bacterium is responsible for a person’s symptoms.

The European STI Guidelines Editorial Board recommends that clinicians investigate better-established causes of urethritis before considering U. urealyticum as the explanation. It also notes that evidence supporting specific treatment regimens for cases attributed to this bacterium remains limited.

This distinction matters because unnecessary testing and treatment can expose people to medication side effects and contribute to antimicrobial resistance, in which bacteria become less responsive to antibiotics.

Antibiotics and treatment decisions

Antibiotics may be considered when a healthcare professional identifies a clinically significant infection and determines that treatment is appropriate. The choice of medicine depends on the diagnosis, the person’s medical history and other relevant factors.

Doxycycline and azithromycin are antibiotics used for certain bacterial infections, including some causes of urethritis. However, they should not be presented as automatic treatments for every positive U. urealyticum test. The appropriate medicine and duration depend on the condition being treated, and evidence for regimens targeting U. urealyticum specifically is limited.

People prescribed antibiotics should follow their clinician’s instructions and seek advice if symptoms persist or return. They should not use leftover medication or take antibiotics without an appropriate assessment.

Penicillin and other beta-lactam antibiotics are generally ineffective against Ureaplasma because these bacteria lack a cell wall, a structure targeted by that class of medicine.

Pregnancy and reproductive health

Researchers have investigated possible links between Ureaplasma and adverse pregnancy outcomes, including preterm birth. The bacterium has also been studied in relation to male infertility and other reproductive health conditions.

However, an association does not prove that the bacterium directly causes a particular complication. The available evidence does not justify claiming that every person with U. urealyticum faces infertility or pregnancy-related complications.

The 2018 European position statement does not recommend routine screening and treatment of pregnant or non-pregnant people simply because U. urealyticum may be present. Pregnancy-related concerns should instead be assessed by an appropriate healthcare professional, who can consider the individual’s symptoms, history and clinical findings.

Newborns with suspected infections require medical assessment. Treatment decisions for newborns and pregnant patients should be made by qualified clinicians rather than based on general antibiotic recommendations.

Can natural remedies treat Ureaplasma?

Claims that neem leaves, tea tree oil or probiotics can eradicate U. urealyticum are not supported by sufficient clinical evidence to recommend them as cures.

Applying concentrated oils or plant preparations to genital tissue may cause irritation. Such remedies should not replace medical assessment or prescribed treatment when a clinically significant infection has been diagnosed.

Probiotics may have uses in some health contexts, but they should not be described as a proven treatment for U. urealyticum.

The main clinical consideration is whether the bacterium is actually causing disease. Because it can be present in healthy people, a positive test should be interpreted alongside symptoms, examination findings and tests for other possible causes rather than treated as proof of an infection.
