Find out how lisinopril lowers blood pressure, how long it stays in the body, its common side effects, drug interactions and important pregnancy warnings.
Prescribed for high blood pressure and heart failure, lisinopril relaxes blood vessels and reduces the heart's workload. Its effects, potential risks and clearance from the body depend partly on individual health factors. Photo/courtesy

Lisinopril begins lowering blood pressure within hours of a dose, but the time needed to achieve the full benefit depends on the condition being treated and the patient’s response to the medicine.

The prescription drug belongs to a class of medicines called angiotensin-converting enzyme (ACE) inhibitors. It is used to treat high blood pressure and heart failure, and to improve survival after certain heart attacks.

Unlike some medicines that provide immediate symptom relief, lisinopril works by relaxing blood vessels, making it easier for the heart to pump blood. Its effects on blood pressure can begin after the first dose, while the full benefit of treatment may take longer to assess.

When does lisinopril reach its peak effect?

The medicine’s concentration in the blood typically reaches its highest level about seven hours after an oral dose, according to prescribing information. However, the peak drug concentration is not the same as the time when blood pressure falls the most. Clinical studies have found that lisinopril’s blood-pressure-lowering effect can begin within about an hour, with the greatest reduction from a single dose often occurring around six hours later.

The response varies between patients. Doctors may start treatment at a lower dose and adjust it over days or weeks according to blood pressure readings, other medical conditions and side effects. Patients should not increase their dose because their blood pressure has not fallen as quickly as expected.

Lisinopril is usually taken once daily. Patients should continue taking it as prescribed and consult their healthcare provider if their blood pressure remains above the target set for them.

What lisinopril treats

Lisinopril is prescribed for several cardiovascular conditions:

  • High blood pressure: It relaxes blood vessels, reducing the pressure against their walls.
  • Heart failure: It can be used alongside other medicines to help manage the condition and reduce the heart’s workload.
  • Heart attack: It can improve survival following certain heart attacks.
  • Some patients with diabetes-related kidney problems: Clinicians may prescribe it to help protect kidney function in appropriate cases.

The drug works by blocking angiotensin-converting enzyme, which helps the body produce angiotensin II, a substance that narrows blood vessels. Blocking this process allows blood vessels to relax and can lower blood pressure.

Side effects and monitoring

Some patients experience no side effects, while others may develop a persistent dry cough, dizziness, headache, diarrhoea or nausea. Dizziness may be more noticeable when standing up, particularly after treatment begins or the dose changes.

Lisinopril can also raise potassium levels in the blood and affect kidney function. Healthcare professionals may order blood tests to check kidney function and potassium, particularly after treatment starts or the dose changes, and when a patient’s medical circumstances increase the risk of complications.

Swelling of the face, lips, tongue or throat can indicate angioedema, a potentially life-threatening reaction. Anyone experiencing these symptoms, especially with difficulty breathing or swallowing, needs emergency medical care.

Drug interactions and conditions to disclose

Patients should tell their doctor or pharmacist about all prescription medicines, over-the-counter drugs and supplements they take before starting lisinopril.

Some combinations can increase the risk of side effects or reduce treatment effectiveness. Diuretics, which help the body remove excess fluid, may increase the risk of low blood pressure. Potassium supplements and potassium-containing salt substitutes can raise potassium levels, while non-steroidal anti-inflammatory drugs (NSAIDs), including ibuprofen, can affect kidney function in some patients taking lisinopril. The medicine can also interact with lithium, potentially increasing lithium levels in the blood.

People with kidney disease, diabetes or a history of angioedema should discuss these conditions with their healthcare provider before treatment. Kidney impairment can slow the removal of lisinopril from the body, affecting drug levels and potentially requiring a different dose.

Dosage depends on the patient

There is no single dose suitable for everyone taking lisinopril. The prescribed amount depends on the condition being treated, blood pressure response, kidney function and other medicines.

For adults with high blood pressure, prescribing information commonly recommends an initial dose of 10 milligrams daily, with adjustments according to response. A maintenance dose of 20 to 40 milligrams daily is commonly used, although starting doses and dosing schedules differ for other conditions and individual circumstances.

Children aged six years and older may also receive lisinopril for high blood pressure, but their doses are determined differently and require medical supervision. Its safety and effectiveness for treating high blood pressure have not been established in children younger than six.

Patients should follow their prescriptions rather than change doses or stop treatment without consulting their healthcare provider.

Pregnancy warning

Lisinopril can harm a developing baby and should not be used during pregnancy. Anyone who becomes pregnant while taking the medicine should contact their healthcare provider immediately for advice on stopping it and switching to an appropriate alternative.

How long does lisinopril remain in the body?

Lisinopril has an effective half-life of about 12 hours during repeated dosing. A drug’s half-life is the time it takes for its amount in the body to fall by half.

The medicine is not substantially broken down by the body. It is excreted largely unchanged through the kidneys, meaning impaired kidney function can delay its removal.

A half-life of 12 hours does not mean the medicine is completely eliminated after 12 hours. Several half-lives are generally needed for most of a drug to leave the body, and the actual time varies between individuals.

Lisinopril is not a typical target of standard illicit-drug screening panels. Its detection depends on the test used, and there is no reliable universal timeline for when it would or would not be detected by a specialised test.

Patients should not stop taking lisinopril simply because they believe it has left their system. The medicine’s ongoing effect on blood pressure and its role in treating heart conditions are separate from how long measurable amounts remain in the body.
